Envisia Plus Plans
Basic Plan
Includes the following services: Two cleanings (prophylaxis) per yearly subscription., necessary x-rays including CBCT, in-office fluoride treatments with cleaning, periodic exams by the doctor, and an emergency exam if needed. In addition, there is an unlimited discount of 20% on all other services (except for Invisalign products). The discount is available after subscription is funded and with payment in full by the patient for dentistry at time of service. The discount does not apply if extended payment plan is needed.
New clients who subscribe to this before or concurrent with their initial comprehensive exam appointment will have that visit covered as part of the plan. Membership may not be applied retroactively.
Periodontal Plans
Same as the basic but will include and third or fourth periodontal maintenance cleaning, and a periodontal examination with the hygienist. Many dental plans only pay for two visits so this may be an exceptionally good deal.
Cost of the Plans
Getting Started
New members are charged 3 months to begin, then monthly starting the fourth month. Kleer members within 60 days of starting only pay 1 month and are then billed monthly.
Basic Plan
Basic $39 per month for 12-month contract. The first three months paid at start for folks not already on Kleer within 60 days of application for Envisia Plus. Payments are made via automatic payment from debit or credit card monthly (4 th month for new members). ($468 total)
If paid up front, one month’s payment will be waived for $429 total. Save $305+
Periodontal Plans
Periodontal 3
3 maintenance visits - $59 per month for 12-month contract. The first three months paid at start for folks not already on Kleer within 60 days of application for Envisia Plus. Payments made via automatic payment from debit or credit card. (4 th month for new members). ($708 total) If paid up front, one month’s payment will be waived for a $649 total. Save $359+.
Periodontal 4
4 maintenance visits - $66 per month. Same terms as Periodontal 3 with 4 cleanings for $792 total ($726 prepay) for savings of $391.
